All candidates must have a minimum of 6 months paid experience within a similar role from the last 3 years
Duties:
- Complete patient assessment and determine their clinical management needs
- Manage patient’s care
- Increase patient involvement in valuable activities in the community
- Administration of medication
- Ensure accurate records are set up and maintained for all contacts
