Care Coordinator / Occupational Therapist: band 6
Location: Oxfordshire
Job role:
- Care of Older adults with mental health needs both functional and dementia
- Assessment of mental health needs, care planning with psychosocial interventions and medication
- Care Coordination
- Car driver essential with access to own vehicle
- Community experience, experience in Older adult mental health is preferred but experienced clinicians with an interest in older adults will be considered.
Job Description:
- Working hours: 9am - 5pm
- ASAP start - Ongoing Contract
- Monday to Friday
- Hourly rate up to £25.00
Requirements:
- HCPC registration
- 6 months NHS experience minimum
- Right to Work in the UK
