Band 7 Clinical Pharmacist - Elderly Medicine

We are currently recruiting for a Band 7 Clinical Pharmacist – Elderly Medicine to work on a temporary basis in Hampshire. Please see details of the role below: Role Details Role: Band 7 Clinical Pharmacist – Elderly Medicine Days: Monday to Friday Hours: 9am to 5pm Pay: £30ph Start: ASAP Length: 3 months This is an excellent opportunity for an experienced Clinical Pharmacist to join a busy elderly medicine team in Hampshire. You will be providing a high-quality clinical pharmacy service across elderly medicine wards, supporting the safe and effective use of medicines for an older patient population. Duties will include medicines reconciliation on admission and discharge, clinical screening of prescriptions, ward-based pharmacy support, polypharmacy reviews and medicines optimisation for elderly patients, participation in multidisciplinary team (MDT) meetings, and contributing to deprescribing initiatives and falls prevention programmes where appropriate. Requirements Current GPhC registration Minimum of 6 months NHS hospital experience Demonstrable experience in clinical pharmacy within an elderly medicine or general medicine setting Strong knowledge of medicines reconciliation, polypharmacy, and medicines optimisation in older patients Familiarity with deprescribing principles and falls prevention medicines management Experience contributing to MDT working Excellent communication and clinical decision-making skills Ability to work autonomously at Band 7 level Why work with Cpl UK Healthcare?
