Outline of Duties and Responsibilities:
· Providing all secretarial and administration to the Diabetes/Endocrine Consultant and the team - managing email accounts, preparing post (referrals/results), all typing, managing diaries for holidays/blocking clinics, organising social events.
· Organise and maintain the Diabetes and Endocrine Outpatient Clinics, making all appointments for new patients/ O.P.D. follow-ups/scheduling testing/cancelling and rescheduling appointments.
· Booking and preparing Virtual Clinic for Diabetes and Endocrine team.
· Taking minutes of meeting – departmental/external.
· Typing all clinic letters and discharge summaries, ensuring all follow-up/testing is organised.
· Queries - answering all telephone queries from patients, internal departments and other hospitals. Pulling charts/getting results/following up on all queries.
· Act as non-clinical contact point for patients, relatives and St. Vincent’s University Hospital
· Manage the Outpatient clinics, organize patients’ appointments and regular updating of outpatient clinic waiting list according to the changing status of patients.
· Liaise with admission staff and wards regarding the admission of patients to SMH. Check to see an updated inpatient list.
· Type letters relating to clinical activities generated by the team, NCHDs and all general Diabetes and Endocrine.
· Organize and collate patient Healthcare Record, letters, laboratory data and patient x-ray’s
· To co-ordinate the delivery of out-patient services to patients attending the Diabetes and Endocrine out-patient clinics.
· Act as a contact and liaison person both between members of the Diabetes and Endocrine staff and between patients and relatives.
· Maintain and input patient’s clinical information on TYMAX (soon to be replaced by Diamond)Applying for Dexcom G6 and G7 CGM (Constant Glucose monitoring) devices from the HSE for our patients. These need to be redone every year as each application with the HSE only lasts for 12 Months.
· Facilitating Zoom calls for the nurses and Dietician.
· Contacting the Retinal Screen programme to make sure that the patient is registered for the programme. (Patients need to have a clear Retinal Screen to be started on Ozempic).
· Reading reports from SYNGO imaging for dictation.
· DAFNE administrator. (Dose adjusting for normal eating). The preparation and facilitation of the DAFNE course. This includes the contact of the patients, gathering their information. Creating education packs for each participant. Helping them get set-up on the Open University site. The setting up and booking of Zoom calls for the course for the DAFNE educators and patients. Uploading of all participants’ information on to the DAFNE database. Booking of post DAFNE 6 week follow-up appointments and annual post DAFNE OPD appointments.
· Care-Link administration. Setting our insulin pump patients on the care-link system for monitoring our Medtronic insulin pump patients.
· Insulin Pump applications for Type 1 Diabetes patients both Medtronic Pumps and Tandem T-Slim pumps.
· The post holder is accountable, responsible and has authority for delivering a quality service and ensuring patient safety. The post holder will work within a Risk Management Framework to achieve the HIQA Safer, Better Healthcare Standards and other quality standards as appropriate.
