Principal Duties and Responsibilities
The Care Coordinator will be responsible for ensuring high quality care and support services are provided to enable individuals to remain in their own homes, living a fulfilled life, which respects their diversity, culture, and values.
The role of the Care Coordinator is to support the delivery of the highest quality care support services in a hands-on way.
Duties of the role will include (but are not limited to) to following:
· Scheduling of Care Workers to deliver care and proactively planning to fill capacity gaps
· To carry out initial visits and appropriate assessments for all new care packages, creating care/support plans and carrying out Risk Assessments
· Liaise with recruitment team to ensure adequate capacity
· Provide reports as requested on care hours delivered and forecast
· Print and distribute documentation for service users including visit sheet
· Maintain regular communication with Care Workers to enable them to discuss any matters regarding their scheduled calls and to provide regular support
· Undertake Medication Competency Observations and training with Care Workers
· To carry out new and current Care & Support Workers field observations
· To carry out investigations into complaints and safeguarding issues and ensure actions are followed through
· To undertake on-call activities as directed by your Line Manager
· To undertake other duties as requested by your Line Manager
· To undertake the use of the PASS & QCS systems effectively
Requirements
· A diploma in Health & Social Care Level 2 (or equivalent).
· Previous experience working as a Care Coordinator, as a supervisor or other role within a care providing organisation.
· Proficiency in the English language, together with excellent communication skills.
· A strong understanding of legislation concerns with care provision.
· A full UK drivers’ licence and access to own transport.
· Eligibility to work in the UK
